Step one click here   open a page like below image. (You must login in your facebook account and you must have to admin privacy the page where want add contact form )

On this click on get started or click here then open a page like below image.

Now select your facebook page where do you want add contact form and click on add Page Tab. Than going to open your facebook page and click on more get contact option look like below image.

Now click on contact open page like below image. Here input a admin email or company support email where to sent email if this any message sent user.

And click on setup account and show “Setup Completed.” Now click on contact again show below page..
